I acknowledge that my child must come to session in the correct uniform. This includes no earrings, jewellery or anything sharpe that might damage the mat.

I allow my child to be spotted and supported by the teachers.

My child will only be allowed to do what the teachers know they are capable of doing safetly.

I hereby acknowledge this release from liability for accidental injury or illness which my child may incur as a result of participating in this activity. I hereby assume all risks connected therewith and consent to participate my child in this program. I agree to disclose my childs physical limitations, disabilities, ailments, or impairments which may affect thier ability to participate in this program.
